I guess being severely underweight is also just a physical condition. But when someone is severely underweight it becomes an urgent medical issue and quite often the patient is diagnosed with a mental disorder like anorexia or bulimia. Unless it's caused by some physical condition like digestion issues or cancer or whatever.
Why doesn't obesity become an urgent medical issue where people are stamped with a mental health issue related to their obesity? Quite often it seems to be the case that obese people are struggling with their mental health which leads into obesity rather than it being a strictly physical condition relating to hormonal imbalance for example.
It is of course easier/faster to die from lack of nutrition than from obesity, but on a societal level medical conditions stemming from obesity are a much bigger problem. Yet the medical field doesn't seem to act accordingly. Obese people should also be admitted to hospitals and put on a strict diet to bring their weight down to a healthy level.
